3.4) physical residential addresses  of directors/ company secretary


3.5) Directorships held in other companies if applicable.


3.6) Email address of the proposed company or email address of one of the directors to be used for business purposes. 


4) Name of the director or person nominated as company secretary


5) Physical business address of the company if it is available or physical residential address of one of the directors to be used as company physical business address.
